1. Know which volunteer role/s you would like to do. Full details are on the
NHS Volunteer
Responders website, and also next to the tick boxes alongside the roles.
2. Know which local authority (council area) you live in. We ask for this because not all
roles are available in every area. When you select your area the available roles will appear
on this form. If your chosen roles do not appear, please register your interest below and
we will contact you if they become available.
3. Photographs of ID documents listed.

- a driving licence

- Utility Bill (last 3 months) + Bank/Credit Card statement (last 3 months)
- Utility Bill (last 3 months + P60

What happens next?
